Using a patient vignette, we will fist assess which alternative options must be considered before initiating insulin in this patient. We will review the barriers to insulin initiation and how to overcome them. The teaching points that need to be adressed with the patient, the self-titration  protocols and the choice of the most appropriate basal insulin will be reviewed in detail.
